dementia care assignment case study

Assignment 3: Case study
Task description The purpose of this assignment is to demonstrate your ability to:
• apply your understanding of aged care service models to a specific case study (ILO4); and
• discuss aged and dementia care service system models (ILO 1).
You will also show your ability to:
• communicate relevant information using appropriate academic writing skills and conventions (ILO5); and may possibly
• discuss perspectives of consumers towards aged and dementia care services (ILO2), though this is not a key focus.
These are all intended learning objectives of the unit.
You will do this by writing an essay about the information in the case study. In the essay you will identify and explain the risks and issues within the case study, and then apply your knowledge of the services available to people living with dementia to help address these identified risks and issues.
This will be done using the case study of Millicent Rathbone.
Preparation:
• Read the case study of Millicent Rathbone. As you read through this case study, make a note of any issues and risks (actual or potential) that you identify.
• Select the service you wish to discuss in your assignment – Alzheimer’s Australia (AA) OR Aged Care Assessment Team (ACAT). (Note that in Victoria this is called the Aged Care Assessment Service).
• Research the service using the AA or ACAT websites and other resources. DO NOT contact either service directly.
• Research appropriate literature to support your work on the UTAS Library website.
Questions:
The following issues should be addressed within your essay:
1. What are the main issues and risks in this case study? Explain why these may impact people with dementia and/or their family? Support your discussion with scholarly literature.
2. What can your chosen service (either AA or ACAT) provide to help Millicent and her family address these issues and risks? Give reasons as to why these services are required and how they can help.
3. If AA or ACAT cannot meet a particular need, which other services might they refer Millicent and her family to? Give reasons as to why these services are required and how they can help.
Writing your assignment:
Main points:
• Always write an introduction and conclusion
• Do not write in the first person
• Write complete sentences and avoid using bullet points
• Paraphrase information you find – avoid direct quotes
• Always provide an in-text citation for all pieces of information you introduce, whether scholarly (for example, when talking about risks and issues or the possible benefits of a service) or websites (ONLY for when talking about a service provider).
After you have got a handle on the ideas and research, write an essay plan, setting out what each paragraph will contain. This will help you form your essay into a logical structure and avoid repetition of information.

Case study – Millicent Rathbone
Millicent Rathbone is 76 years old. She lived with her female partner Elizabeth for 43 years, until Elizabeth’s death from cancer six years ago. Three years ago, Millicent had a series of transient ischaemic attacks (TIAs). Before this time, her family describe Millicent as a gentle-natured woman who frequently travelled around her home state playing bridge. After the TIAs, her family noticed that she was having difficulty finding words when engaging in conversation and was finding it hard to concentrate when playing bridge, which she had to give up along with the travel. Millicent’s GP diagnosed her with vascular dementia. Millicent also has Type 2 diabetes for which she is on medication.
Millicent is currently being cared for by her younger sister Judith. Judith has some support from Millicent’s niece who lives in another town some distance away but visits on most weekends. Judith moved in with Millicent three years ago when Millicent started forgetting to take her diabetes medication, and had several hospitalisations as a result.
Millicent’s niece and Judith report that Millicent’s behaviour has changed in the last year or so. Millicent is becoming irritable and on several occasions has been verbally abusive towards Judith, which Judith finds distressing. Judith has described Millicent’s mood swings as being the worst in the evening and she has been woken in the night by hearing Millicent moving around. Millicent’s irritability extends to her taking her prescribed medication. At times Millicent refuses to take her medication for her diabetes, which is increasing her diabetes symptoms.
Millicent sustained a small wound to her left lower leg when she bumped it on a coffee table in the lounge room. Judith has been putting dressings of gauze on the wound, but she reports that after nearly three weeks it does not seem to be getting any better and she would like a health professional to give her some advice about the best way to look after it.
Millicent has also had trouble finding the toilet. On several occasions, Millicent has used a flower pot in the backyard to urinate. On occasion, Millicent has also wet her clothing. This problem is increased by Millicent’s frequent need to urinate as a symptom of her worsening diabetes.
Note: Please assume that Millicent lives in the state/territory in which you live.
