Answer all parts of each question. For example, question # 1, asks you why the medications are ordered for the client and the therapeutic effect of the medication. To get full marks, list the first medication, state why it is ordered for this patient (1 mark), and its therapeutic effect. (1 mark). For example, a patient might be on Lasix for high blood pressure. The therapeutic effect of Lasix is that it lowers blood pressure by inhibiting the reabsorption of sodium and increases the excretion of water. That same patient might also be on Captopril to reduce high blood pressure. The therapeutic effect of Captopril is that it blocks the conversion of angiotension I to angiotension II thereby preventing vasoconstriction from occurring. The dilated vessels decrease resistance to blood flow and lower blood pressure. Repeat for all of the medications ordered for the patient.
Question # 2 asks you to indicate safe dosage (1 mark per medication) then asks if the dosage ordered is safe. You must indicate whether or not the dosage is safe for each of the medications (1 mark per medication).
Question # 5 asks What primary assessments are required prior to administration of these medications? You are to describe the assessments that are required prior to the administration of each of the medications. For example: Nardil is an MAOI antidepressant. Prior to administering a dose, the nurse should assess the patient’s vital signs, assess for feelings of depression and thoughts of suicide, and for dizziness.
Question # 6 asks What are the nursing implications associated with each medication? In other words, what are the ongoing assessments that would be required for a patient on these medications? To continue with the MAOI example, the nurse would monitor for orthostatic hypotension (vertigo when changing position), gastrointestinal upset (nausea, diarrhea, dry mouth, constipation), genitourinary reaction (impotence) and hypertensive crisis (occipital headache, stiff neck, nausea, vomiting, sweating, fever, chest pain, dilated pupils, and change in heart rate), as well as thoughts of depression and suicide. The nurse must also monitor the client’s diet to ensure that it does not contain tyramine.
